Request for Direct Service Grant Applications

We are still in this together and the Florida Breast Cancer Foundation (FBCF) is still here for YOU. We were proud to offer the COVID-19 Emergency Assistance Fund through our Direct Service program in August in response to the pandemic, and now we continue our regular funding cycle to offer two funding opportunities to nonprofits to:

1) Provide living expenses to individuals in active treatment for breast cancer

2) To assist individuals in Broward County in need of Tomosynthesis (3D) Diagnostic Mammograms, reaching as many county residents as possible. 

 

 

STATEWIDE

DIRECT SERVICE GRANT FOR LIVING EXPENSES

This RFA invites applications from any Florida-based 501 (c)(3) organization providing direct service support to breast cancer patients in active treatment. The Florida Breast Cancer Foundation anticipates awarding up to ten (10) awards of up to $5,000 maximum each, to organizations to provide living expenses to individuals in active treatment for Breast Cancer, dependent on quality of applications received and budget availability. The aim of this initiative is to reach as many Floridians as possible.

Areas of Interest

The intention of this award is to provide funding to organizations providing living expenses to breast cancer patients in active treatment. Services of interest for this RFA include, but are not limited to:

  • Travel
  • Groceries
  • Child Care
  • Rent / /Mortgage
  • Utilities
  • Household Cleaning
  • Yard Maintenance

Restrictions

  • Services provided can only be for living expenses for Floridians in active treatment for Breast Cancer.
  • Direct Service grantees are limited to providing individuals up to a maximum of $500 in awards.
  • Maximum award per organization may not exceed $5,000.
  • Organizations may have only one active award in any grant period. Active awards must end before a new award may be issued to an organization in the current grant period.

Eligibility: Only Florida non-profit community-based organizations that are tax-exempt under Internal Revenue Code section 501(c)(3) and Florida governmental entities are eligible to apply for funding, e.g., Florida tax-exempt non-profit organizations, Florida tax-exempt educational institutions, Florida governmental agencies, and Indian tribes within the State of Florida. US citizenship or residency is not required. Applications must be submitted in English.

BROWARD COUNTY ONLY

DIRECT SERVICE LIMITED FUNDING OPPORTUNITY IN BROWARD COUNTY FOR TOMOSYNTHESIS (3D) DIAGNOSTIC MAMMOGRAMS 

This Limited Opportunity RFA invites applications from Broward County-based 501 (c)(3) organizations providing Tomosynthesis (3D) Diagnostic Mammograms to residents of Broward County. The Florida Breast Cancer Foundation anticipates awarding at least three (3) awards of up to $10,000 maximum each, dependent on quality of applications received and budget availability. The aim of this funding opportunity is to assist individuals in Broward County in need of Tomosynthesis (3D) Diagnostic Mammograms, reaching as many county residents as possible.

Restrictions

  • Service provided can only be for Tomosynthesis (3D) Diagnostic
  • Requested funds for Tomosynthesis (3D) Diagnostic Mammograms cannot be greater than the standard reimbursement rate provided by Medicare per
  • Maximum award per organization may not exceed $10,000.
  • Organizations may have only one active award in any grant Active awards must end before a new award may be issued to an organization in the current grant period.
  • Services must be provided in Broward
